Our team reviews your records before scheduling to ensure you qualify under Washington State guidelines.

Complete the booking form with your preferred location, contact information, and medical history details.
Submit your medical records
Send your qualifying medical records by upload, email, or fax so our team can review whether you meet Washington State qualifying conditions.
Qualification & appointment preparation
Once qualified, complete the consent forms and submit your valid ID before your appointment is confirmed.
Telehealth is only available for renewal patients who have previously been evaluated by one of our providers. New patients must first be seen in person.
